Leah CalvertTeen Mom 2 is coming back to our small screens next week for for season 5B, and it looks like we're going to need to break out the tissues. We've watched Leah Calvert and Corey Simms struggle with their disabled daughter Ali's condition since infancy, and now that they finally have some answers and Ali is growing up, she's starting to understand that she's "different."

In a tear-jerking sneak peek, Leah and Corey chat about the kids as they make their co-parent exchange. Ali has a rare developmental disorder called Titin Muscular Dystrophy, which causes degenerative physical weakness. This is first time we've seen Ali acknowledge the disease.

Corey tells Leah that Ali knows she has something that makes her stand out from other kids.

"The other day when we got her home, she said, 'Daddy, I'm not strong because I have Muscle Dystrophy. And I can't do a lot of things.' I don't know what 4-year-old knows that," Corey tells Leah. "I don't understand how she would all of a sudden tell me that. I about lost it."

Oh my gosh, break our hearts, why don't you? Of course she didn't actually say dystrophy, but something that sounded like it, and I'm just glad the cameras didn't catch that because weep!

Ali has such great parents though. Leah was taken aback at first but is determined that her little girl doesn't see herself as weak. "Right now she's young and she's gonna play pity party," Leah said. "We are her parents to lift her up. You fall; you get back up. You keep doing it."

Yup, definitely going to have to bring the tissues for the Teen Mom 2 return on July 16.
