Do you take a photo of your kids on the first day of school every year? Create more memories by making a questionnaire for each of your kids to take. Keep a collection as they continue to fill it out the questions year after year. You can compile them into a book including the photo and survey. I think I'd use a binder and put the first on the first page- saving room for the pictures along with the answers.

I like all the questions you have, so I would of course keep them all. The only thing I could think to add is to ask who their teacher for the year was. :)
